Confusion over EHR use stems from poor design

Dr. David M. Polaner, Associate Professor of Anesthesiology and Pediatrics at The Children's Hospital, University of Colorado School of Medicine in Aurora wrote an interesting editorial in Modern Healthcare online.  Please read.  The letter questions if EHRs are really meeting the needs of physicians, specifically he questions the way in which encounters are documented.  Much of topics we discuss on this blog elude to the fact that hybrid solutions where practices decide to integrate transcription with their EMR results in optimal productivity, quality and patient intimacy.
